Download our registration form as a fillable PDF directly from this website.  Or, printed copies are available in the Welcome Center in the rear of church (Center Street), and at the parish office adjacent to the church. The Welcome Center is staffed after Mass the second weekend of each month.

The parish office is open 8:00 a.m. to 3:30 p.m. Monday through Friday, except during summer months when it closes at noon on Friday. The office is closed on Holy Days of Obligation and certain federal holidays.

Information needed for the registration form includes names of family members, contact information, sacramental information (date and place of Baptism, Confirmation, Marriage, etc.), occupation, education and marital status.
